Apto Options provides a coordinated integrated approach to Community Care for older persons and younger people with disabilities. This includes Care Management and support to access the full range of Home Care services needed to allow people with complex care requirements to remain in their own home for as long as possible.

Apto Options is available to support Health Services and their patients requiring post hospital home care. The Apto Options Team includes experienced Allied Health professionals and specialist Community Nurses.
